Your First Visit

Thank you for choosing our practice! We are committed to becoming a valued part of your dental team providing you complete dental care. Your consultation appointment is to introduce ourselves and to comprehensively evaluate and discuss your situation.

Once your situation has been evaluated and the treatment selected and explained to your satisfaction our office will assist you step by step until the completion of your care. Please keep in mind this enables us to tailor your care to your specific needs and requirements. This visit can simplify the surgical experience for you and improve your healing phase care.

To maximize your visit please assist us by providing the following information at the time of your consultation:

  • Your surgical referral slip and any current X-rays if applicable
  • A list of medications you are presently taking
  • If you have medical or dental insurance, bring the necessary completed forms. This will save time and allow us to help you process any claims.(On line Forms link)

IMPORTANT: So as not to inconvenience you, all patients under the age of 18 years of age must be accompanied by a parent or guardian at the consultation visit.

A consultation and physical examination is mandatory for patients undergoing general anesthesia or sedation for any type of surgery. Most consultations can be completed on the same date as your surgery. Please alert the office if you have a medical condition that may be of concern prior to surgery (i.e. diabetes, high blood pressure, artificial heart valves and joints, rheumatic fever, etc.) or if you are currently taking any medications (i.e. heart medications, aspirin, anticoagulant therapy, etc.).

X-Rays

If your dentist or physician has taken x-rays, you may request that they forward them to our office. If there is not enough time, please pick them up and bring them to our office. Please be aware a current x-ray or panorex (less than one year old) is often required to evaluate many oral surgical conditions. If additional films are necessary, they can be taken at our facility.
